Habiendo culminado el 22/06/2022 la etapa de planificación con el establecimiento de metas y suscripción del formato de la Gestión del Rendimiento (GDR), se dio inicio a la etapa de seguimiento de metas (23/06/2022 al 22/12/2022), desarrollándose el pasado 7 y 8 de julio cuatro talleres virtuales, dónde se explicó temas como las reuniones de seguimiento, la documentación de evidencias, el cálculo del avance de las metas de desempeño y las oportunidades de mejora

RESUMO Objetivo: verificar a associação entre peso ao nascer, idade gestacional e diagnósticos médicos secundários no tempo de permanência hospitalar de recém-nascidos prematuros. Métodos: estudo transversal, com 1.329 prontuários de recém-nascidos no período de julho de 2012 a setembro de 2015, em dois hospitais de Belo Horizonte, que utilizam o sistema Diagnosis Related Groups Brasil. Para determinar um ponto de corte para o peso ao nascer e a idade gestacional no nascimento que melhor determinasse o tempo de internação, foi utilizada a curva Receive Operator Characteristic. Posteriormente, utilizou-se o teste de análise de variância e teste de Duncan para a comparação entre a média de tempo de permanência hospitalar. Resultados: a prematuridade sem problemas maiores (DRG 792) foi a categoria mais prevalente (43,12%). O maior tempo médio de internação foi de 34,9 dias, identificado entre os recém-nascidos prematuros ou com síndrome da angústia respiratória (DRG 790). A combinação de menor peso ao nascer e menor IG ao nascimento apresentou o maior risco de permanência hospitalar, aumentada quando comparados aos demais perfis formados para esse DRG. Conclusão: os achados poderão direcionar a assistência em relação à mobilização de recursos físicos, humanos e de bens de consumo, além da análise crítica de condições que influenciam os desfechos clínicos. A possibilidade da otimização do uso desses recursos hospitalares aliada à melhoria da qualidade dos atendimentos e da segurança dos pacientes está associada à minimização do tempo de permanência hospitalar e da carga de morbidade e mortalidade neonatal.


RESUMEN Objetivo: verificar la asociación entre el peso al nacer, la edad gestacional y los diagnósticos médicos secundarios en la duración de la estancia hospitalaria de los recién nacidos prematuros. Métodos: estudio transversal, con 1.329 registros de recién nacidos de julio de 2012 a septiembre de 2015, en dos hospitales de Belo Horizonte, que utilizan el sistema Diagnosis Related Groups Brasil. Para determinar un punto de corte para el peso al nacer y la edad gestacional al nacer que mejor determina la duración de la estadía, se utilizó la curva Receive Operator Characteristic. Posteriormente, se utilizó la prueba de análisis de varianza y la prueba de Duncan para comparar la duración media de la estancia hospitalaria. Resultados: la prematuridad sin mayores problemas (DRG 792) fue la categoría más prevalente (43,12%). La estancia media más larga fue de 34,9 días, identificada entre los recién nacidos prematuros o aquellos con síndrome de dificultad respiratoria (DRG 790). La combinación de menor peso al nacer y menor IG al nacer presentó el mayor riesgo de estancia hospitalaria, que se incrementó en comparación con los otros perfiles formados para este DRG. Conclusión: los hallazgos pueden orientar la atención en relación con la movilización de recursos físicos, humanos y de bienes de consumo, además del análisis crítico de las condiciones que influyen en los resultados clínicos. La posibilidad de optimizar el uso de estos recursos hospitalarios, aliada a mejorar la calidad de la atención y la seguridad del paciente, está asociada a minimizar la duración de la estancia hospitalaria y la carga de morbilidad y mortalidad neonatal.


ABSTRACT Objective: to verify the association between birth weight, gestational age, and secondary medical diagnoses in the length of hospital stay of premature newborns. Methods: cross-sectional study, with 1,329 medical records of newborns from July 2012 to September 2015, in two hospitals in Belo Horizonte, which use the Diagnosis Related Groups Brasil system. To determine a cutoff point for birth weight and gestational age at birth that best determined the length of hospital stay, the Receive Operator Characteristic curve was used. Subsequently, the analysis of variance test and Duncan's test were used to compare the mean length of hospital stay. Results: prematurity without major problems (DRG792) was the most prevalent category (43.12%). The longest mean length of hospital stay was 34.9 days, identified among preterm infants or infants with respiratory distress syndrome (DRG 790). The combination of lower birth weight and lower GA at birth presented the highest risk of hospital stay, increased when compared to the other profiles formed for this DRG. Conclusion: the findings may direct assistance in relation to the mobilization of physical, human and consumer goods resources, in addition to the critical analysis of conditions that influence clinical outcomes. The possibility of optimizing the use of these hospital resources, allied to improving the quality of care and patient safety, is associated with minimizing the length of hospital stay and the burden of neonatal morbidity and mortality.

INTRODUCCIÓN Y OBJETIVO: Analizar la evolución y los costes de los partos prematuros iatrogénicos en un hospital universitario de tercer nivel. MÉTODOS: Estudio de cohortes retrospectivo de los partos con edad gestacional comprendida entre 24 y 36+6 semanas en dos periodos temporales: 2001-2005 y 2011-2016. Se identificaron los partos prematuros por indicación médica o iatrogénicos (PPI). Se analizaron variables demográficas y de resultado. Los costes se calcularon mediante el grupo relacionado con el diagnóstico (GRD) de cada ingreso. RESULTADOS: Se obtuvo una muestra de 620 partos prematuros iatrogénicos. La tasa de prematuridad global se mantuvo estable en 9%. La tasa de prematuridad iatrogénica experimentó un incremento relativo del 9,7%. Entre las pacientes con un PPI se apreció un incremento en la edad materna de 27,7 a 32,9 años, de la obesidad (32,2% a 55,5%) y del uso de técnicas de reproducción asistida (6% a 11,1%). Preeclampsia y retraso del crecimiento (CIR) fueron las principales causas de PPI, en los que se incrementó la tasa de cesáreas de 66,9% a 78%, la estancia media de 7,8 días a 9,6 y el coste por paciente de 3.068,6 a 7.331,9 euros. CONCLUSIONES: Se observa un aumento de PPI en el segundo periodo, manteniéndose la prematuridad global. Los cambios demográficos podrían explicar este incremento. Estos cambios están implicados en la fisiopatología de la preeclampsia y el CIR y en el aumento de su incidencia. La prevención primaria y secundaria de esas complicaciones podría reducir la incidencia y los costes de la prematuridad iatrogénica.


INTRODUCTION AND OBJECTIVES: To evaluate the incidence, evolution, causes and costs of premature births (PB) due to medical indication (iatrogenic) in a tertiary care university hospital METHODS: Retrospective cohort study of all deliveries with gestational age between 24 and 36+6 weeks, in two periods 2001-2005 and 2011-2016. Iatrogenic births were identified. Clinical, epidemiological, diagnostic and economic variables were analysed. RESULTS: A sample size of 620 iatrogenic deliveries was obtained. We found a 9.7% relative increase in iatrogenic prematurity rate in the second period as well as an increase in maternal age from 27.7 to 32.9, obesity from 32.2% to 55.5% and the use of assisted reproductive techniques from 6% to 11.1%. Preeclampsia and intrauterine growth restriction were found to be the main causes of iatrogenic premature delivery. In these cases the rates of cesarean section increased from 66.9% to 78%. The average stay per patient and the cost calculated by diagnosis related group (DRG) also showed a statistically significant increase from 7.8 days and 3,068.6 euros to 9.6 days and 7,331.9 euros. CONCLUSIONS: We observed an increase in iatrogenic prematurity in the second period despite the unchanged rate of spontaneous PB. Demographic changes in the population, as well as an increase in obstetric related conditions, seem to be responsible for this increase. Primary and secondary prevention of clinical characteristics may reduce the incidence and costs derived from this type of prematurity.

Abstract Cases of maternal near miss are those in which women survive severe maternal complications during pregnancy or the puerperium. This ecological study aimed to identify the temporal trend of near-miss cases in different regions of Brazil between 2010 and 2018, using data fromtheHospital Information System(HIS) of theUnified BrazilianHealth System (SUS, in the Portuguese acronym). Hospital admission records of women between 10 and 49 years old with diagnosis included in the 10th Revision of the International Statistical Classification of Diseases and Related Health Problems (ICD-10) and codes indicating nearmiss events were selected. From 20,891,040 admissions due to obstetric causes, 766,249 (3.66%) near-miss cases were identified, and 31,475 women needed admission to the intensive care unit (ICU). The cases were found to be more predominant in black women over 35 years old from the North and Northeast regions. There was a trend of increase in near-miss rates of ~ 13.5% a year during the period of the study. The trend presented a different behavior depending on the level of development of the region studied. The main causes of near miss were preeclampsia (47%), hemorrhage (24%), and sepsis (18%).


Resumo Casos de near miss materna são aqueles em que as mulheres sobrevivem a graves complicações maternas durante a gravidez ou o puerpério. Este estudo ecológico teve comoobjetivo identificar a tendência temporal de casos de near missemdiferentes regiões do Brasil entre 2010 e 2018, utilizando dados do Sistema de Informações Hospitalares (SIH) do Sistema Único de Saúde (SUS). Foram selecionados registros de internação demulheres entre 10 e 49 anos comdiagnóstico incluído na 10ª revisão daClassificação Internacional de Doenças e Problemas Relacionados à Saúde (CID-10) e códigos indicando eventos de near miss. Das 20.891.040 internações por causas obstétricas, 766.249 (3,66%) casos de near miss foram identificados, e 31.475mulheres necessitaramde internação na unidade de terapia intensive (UTI). Constatou-sequeos casos sãomaispredominantesemmulheres negras commais de 35 anos da região Norte e Nordeste. Houve uma tendência de aumento nas taxas de near miss de aproximadamente 13,5% ao ano durante o período do estudo. A tendência apresentou um comportamento diferente, dependendo do nível de desenvolvimento da região estudada. As principais causas de near miss foram pré-eclâmpsia (47%), hemorragia (24%), e sepse (18%).

ABSTRACT

El tabaco es la principal causa modificable de enfermedad cardiovascular, cáncer y enfermedades respiratorias, por lo que es un serio problema de salud pública universal. En 2006, Uruguay implementó el Convenio Marco para el Control del Tabaco de la Organización Mundial de la Salud (CMCT-OMS), y consiguió un descenso de la tasa de fumadores y mejoras en la salud cardiovascular y respiratoria. Se investigó el impacto clínico y económico de las medidas de control de tabaco sobre los costos asistenciales del infarto agudo de miocardio, que se redujo un 17%. No se incluyeron los costos evitados por otras patologías. Se investigó lo ocurrido en una institución sanitaria y se proyectó a toda la población del país. El análisis de costos se realizó usando la metodología del sistema de los Grupos Relacionados por el Diagnóstico (GRD), combinada con los informes contables de la institución. Además de los costos de internación, se incluyeron los relacionados con los traslados, los de los procedimientos cardiovasculares invasivos y los gastos sanitarios de los 12 meses posteriores al evento agudo. El costo por paciente fue de USD 12.037. Considerando un descenso de 500 infartos por año, el ahorro anual estimado es de USD 6.000.000 en costos asistenciales de los infartos evitados; a lo que debería agregarse otros ahorros de costos por pérdida laboral, discapacidad posterior y años de vida prematura perdidos por fallecimiento del paciente. La exitosa política de control del tabaco ha sido la principal medida de salud pública en los últimos 30 años en nuestro país. Esta investigación busca contribuir al camino trazado por la Organización Mundial de la Salud (OMS).


Tobacco is the leading modifiable cause of cardiovascular disease, cancer, and respiratory diseases and is thus a serious global public health problem. In 2006, Uruguay implemented the World Health Organization Framework Convention on Tobacco Control (WHO-FCTC) and achieved a decrease in the smoking rate and improvements in cardiovascular and respiratory health. We analyzed the clinical and economic impacts of tobacco control measures on the healthcare costs for acute myocardial infarction, which was reduced by 17%. The costs avoided for other diseases were not included. The study examined the trend in a healthcare institution and projected the result to the country's population. The cost analysis used the diagnosis-related groups (DRG) methodology, combined with the institution's accounting reports. Besides the hospitalization costs, the analysis included patient transportation, invasive cardiovascular procedures, and healthcare costs for the 12 months following the acute myocardial infarction. The cost per patient was USD 12,037. Considering a decrease of 500 acute myocardial infarctions per year, the estimated annual savings are USD 6 million in medical care costs for the averted acute myocardial infarctions, besides savings from averted work absenteeism, subsequent disability, and disability adjusted life years. This successful tobacco control policy has been the leading public health intervention in the last 30 years in Uruguay. The study aims to contribute to the guidelines determined by the World Health Organization (WHO).


O tabaco é a principal causa modificável de doença cardiovascular, câncer e doenças respiratórias, o que faz dele um sério problema de saúde pública universal. Em 2006, Uruguai implementou a Convenção-Quadro para o Controle do Tabaco da Organização Mundial da Saúde (CQCT-OMS), registrando desde então uma queda na taxa de fumantes, além de melhoras na saúde cardiovascular e respiratória. Foi avaliado o impacto económico e clínico das medidas de controle do tabaco sobre os custos da atenção ao infarto agudo do miocárdio, que diminuíram 17%. Não foram computados os custos evitados de outras patologias. A pesquisa investigou os registros em uma unidade de saúde e operou uma projeção para toda a população do país. A análise de custos foi desenvolvida usando a metodologia do sistema de Grupos Relacionados pelo Diagnóstico (GRD), combinada com os relatórios contábeis da instituição. Além dos custos de internação, foram incluídos aqueles relativos aos traslados, aos procedimentos cardiovasculares invasivos e às despesas em saúde dos 12 meses posteriores ao evento agudo. O custo por paciente foi de USD 12.037. Considerando uma diminuição de 500 infartos por ano, a economia anual estimada representa USD 6.000.000 na atenção aos infartos evitados; devem ser acrescentadas outras economias de custos de licença médica, invalidez posterior e anos de vida prematura perdidos por óbito do paciente. A bem-sucedida política de controle de tabaco foi a principal medida de saúde pública nos últimos 30 anos, no nosso país. Esta pesquisa pretende contribuir com o caminho trilhado pela Organização Mundial da Saúde (OMS).

O objetivo desta pesquisa é analisar a associação de variações temporais dos agravos e das mortes por causas externas com períodos de maior fluxo de visitantes em municípios turísticos do litoral de São Paulo, Brasil, no período de 2004 a 2014. Trata-se de um estudo ecológico, de séries temporais diárias e mensais, baseado nos dados do Sistema de Informações sobre Mortalidade (SIM) e Sistema de Informações Hospitalares do Sistema Único de Saúde (SIH/SUS) no período de 1º de janeiro de 2004 a 31 de dezembro de 2014, tendo como unidades de análise os quatro municípios do litoral norte do Estado de São Paulo. Modelos de regressão binomial negativa foram ajustados para o desfecho que representou o número de ocorrências de internações hospitalares e/ou óbitos por causas externas, tendo como preditores as variáveis de calendário (dias da semana, feriados, mês e ano de ocorrência). Os feriados de Ano Novo, Carnaval, Tiradentes e Proclamação da República destacaram-se com médias iguais ou superiores a 5 desfechos por dia. Dentre os dias da semana, a segunda-feira e o domingo apresentaram as maiores médias. Levando em consideração todos os preditores, verifica-se maior tendência de ocorrência dos desfechos na segunda-feira (OR = 1,11; IC95%: 1,05-1,18), nos feriados de Ano Novo (OR = 1,44; IC95%: 1,19-1,74) e Proclamação da República (OR = 1,49; IC95%: 1,13-1,94) e nos meses de janeiro (OR = 1,11; IC95%: 1,02-1,20) e fevereiro (OR = 1,13; IC95%: 1,04-1,23). A morbidade e a mortalidade por causas externas nos municípios estudados foram maiores no período de maior movimentação de turistas, subsidiando a estruturação do atendimento destas lesões e as ações de prevenção e de promoção de saúde direcionadas para este grupo de causas.


This study aims to analyze the association between temporal variations in injuries and deaths from external causes and periods of greater flow of visitors in tourist municipalities (counties) on the coastline of the state of São Paulo, Brazil, from 2004 to 2014. This is an ecological study of daily and monthly time series based on data from the Brazilian Mortality Information System (SIM) and Brazilian Hospital Information System of the Brazilian Unified National Health System (SIH/SUS) from January 1, 2004, to December 31, 2014, using as the analytical units the four municipalities on the northern coast of the State of São Paulo. Negative binomial regression models were adjusted for the outcome that represented the number of hospital admissions and/or deaths from external causes, with calendar variables as predictors (days of the week, holidays, month, and year of occurrence). The Brazilian holidays New Year's Day, Carnival, Tiradentes, and Proclamation of the Republic stood out with mean numbers greater than or equal to 5 outcomes per day. Among the days of the week, Monday and Sunday had the highest mean numbers. Considering all the predictors, there was a higher tendency to the occurrence of outcomes on Monday (OR = 1.11; 95%CI: 1.05-1.18), New Year's Day (OR = 1.44; 95%CI: 1.19-1.74), and Proclamation of the Republic Day (OR = 1.49; 95%CI: 1.13-1.94) and in the months of January (OR = 1.11; 95%CI: 1.02-1.20) and February (OR = 1.13; 95%CI: 1.04-1.23). Morbidity and mortality from external causes in these tourist towns were higher in the period with the greatest flow of tourists, emphasizing the need to organize care for these injuries, alongside measures for prevention and health promotion targeted to this group of causes.


El objetivo de esta investigación es analizar la asociación de variaciones temporales en enfermedades y muertes por causas externas, durante períodos de mayor flujo de visitantes en municipios turísticos del litoral de São Paulo, Brasil, durante el período de 2004 a 2014. Se trata de un estudio ecológico, de series temporales diarias y mensuales, basado en los datos del Sistema de Información sobre Mortalidad (SIM) y Sistema de Informaciones Hospitalarias del Sistema Único de Salud (SIH/SUS), durante el período del 1º de enero de 2004 al 31 de diciembre de 2014, teniendo como unidades de análisis los cuatro municipios del litoral norte del Estado de São Paulo. Modelos de regresión binomial negativa se ajustaron al resultado que representó el número de ocurrencias de internamientos hospitalarios y/u óbitos por causas externas, teniendo como predictores variables de calendario (días de la semana, festivos, mes y año de ocurrencia). Los festivos de Año Nuevo, Carnaval, Tiradentes y Proclamación de la República se destacaron con medias iguales o superiores a 5 resultados por día. Entre los días de la semana, el lunes y el domingo presentaron las mayores medias. Teniendo en consideración todos los predictores, se verifica una mayor tendencia de ocurrencia de los resultados: los lunes (OR = 1,11; IC95%: 1,05-1,18), festivo de Año Nuevo (OR = 1,44; IC95%: 1,19-1,74) y Proclamación de la República (OR = 1,49; IC95%: 1,13-1,94) y, en los meses de enero (OR = 1,11; IC95%: 1,02-1,20) y de febrero (OR = 1,13; IC95%: 1,04-1,23). La morbilidad y mortalidad por causas externas en los municipios estudiados fueron mayores en el período de mayor movimiento de turistas, apoyando la organización de la atención a esas lesiones, así como acciones de prevención y promoción de salud dirigidas hacia este grupo de causas.

INTRODUCCIÓN: Los sistemas de salud se desarrollan en ámbitos complejos y con fallas constantes (incertidumbre, asimetría de información, problema de relación de agencia e inducción de demanda). Estas fallas determinan las relaciones e incentivos entre los actores y se basan en la imperfección del sector. Frente a ello, los mecanismos de pago regulan aspectos del comportamiento e incentivos del sistema, participando como instrumentos de compra de atenciones de salud a prestadores, mediados por los seguros de salud en representación de los usuarios. OBJETIVO: Caracterizar los elementos básicos de las tipologías más frecuentes de los mecanismos de pago con el propósito de para apoyar la labor de los prestadores en su relación con pagadores. MÉTODOS: Se condujo una revisión dirigida de la evidencia en PubMed, Google, Google Scholar y selección estratégica en bola de nieve. Los mecanismos de pago están conformados por tres variables microeconómicas clásicas fijas o variables: precio, cantidad y gasto; y dimensiones temporales utilizadas para analizar sus atributos y efectos. De la combinación de estas variables surgen distintos mecanismos. RESULTADOS: Entre los más utilizados se describen: pago por servicio, presupuesto global, Bundled Payments, grupos relacionados de diagnóstico, per cápita, pago por desempeño y acuerdos de riesgo compartido. Dentro de sus variables ha cobrado importancia una cuarta, el riesgo financiero. CONCLUSIONES: Los mecanismos de pago resultan esenciales para concatenar esfuerzos sanitarios con la práctica clínica. Permiten regular relaciones entre seguros, prestadores y usuarios. Dependiendo de la arquitectura del mecanismo, estas pueden tornarse beneficiosas o entorpecer el cumplimiento de los objetivos del sistema sanitario.


INTRODUCTION: Healthcare systems are developed in imperfect scenarios, in which there are constant failures (uncertainty, information asymmetry, agency relationship problem, and supply-induced demand). These failures, based on the imperfection of the sector, determine the relationships and incentives between the actors. It is within this context that payment mechanisms regulate aspects of the system behavior and incentives, acting as instruments for the purchasing of health care from providers, mediated by health insurance on behalf of users. OBJECTIVE: To characterize the basic elements of most frequent payment mechanisms to help providers in their relationship with payers. METHODS: A review of the evidence was conducted in PubMed, Google, Google Scholar, and strategic snowball selection. Payment mechanisms consist of three classical microeconomics variables, fixed or variable: price, quantity, and expense. Time dimensions are used to analyze their attributes and effects. Different mechanisms emerge from the combination of these variables. RESULTS: Among the most used are: Fee-For-Service, Global Budget, Bundled Payments, Diagnosis-Related Groups, Per-capita, Performance Pay, and Risk-Sharing Agreements. A fourth has also gained importance: Financial Risk. CONCLUSIONS: Payment mechanisms are essential to link health efforts with clinical practice. They make it possible to regulate relationships between insurers, providers, and users, which, depending on the architecture of the mechanism, can become beneficial or hinder the fulfillment of the objectives of the health system.

INTRODUCCIÓN: La gestión actual de camas hospitalarias sigue un modelo de indiferenciación en el que existen camas quirúrgicas, médicas y de pacientes críticos. Las maternidades si bien no siguen este modelo, tienen egresos indiferenciados con pacientes que pueden egresar post parto o aún embarazadas (con patologías perinatales). OBJETIVO: Evaluar diferencias entre egresos con parto normal y aquellos con patologías de alto riesgo obstétrico (ARO) respecto a estancia media (EM) y el consumo de recursos cuantificado con el peso medio de los grupos relacionados con el diagnóstico (PMGRD). MÉTODO: Estudio transversal con egresos maternales periodo 2017-2018. Se usó Categoría Diagnóstica Mayor (CIE - 10) para definir dos grupos de egreso: 1. Con parto normal a término o 2. Con patología ARO. Se compararon 1.658 y 1.669 egresos del grupo 1 y 2 respectivamente. Las variables de resultado son EM y PMGRD. Se compararon variables cuantitativas con t de student y Kruskal Wallis. Se usó Odds Ratio con respectivo intervalo de confianza para evaluar asociación entre variables y regresión logística multivariada para ajustar asociación. RESULTADOS: La edad, proporción de gestantes tardías, EM y PMGRD fue mayor en los egresos ARO (p<0,05). Existe fuerte asociación de EM prolongada (>4 días) y PMGRD elevado (>0.3109) con los egresos ARO (ORa=3.75; IC95%=3.21-4.39 y ORa=1.28; IC95%=1.1-1.49 respectivamente). CONCLUSIONES: Es necesario diferenciar los egresos del servicio de maternidad porque los egresos de ARO muestran mayor complejidad. La evaluación del riesgo usando EM y PMGRD permite analizar con especificidad los egresos maternales para una mejor gestión de camas y del recurso humano.


INTRODUCTION: The current management of hospital beds in Chile follows an undifferentiation model in which there are surgical, medical and critical patient bed. Maternity hospitals although they do not follow this model, have undifferentiated discharges with patients who may leave poatpartum or still pregnant (with perinatal pathologies) OBJECTIVE: To assess differences between discharges with normal delivery and those with high obstetric risk pathologies (HOR) with respect to mean stay (MS) and the resource spending quantified with the average weight of the diagnosis related group (AWDRG). METHOD: Cross-sectional study with maternal discharges between 2017-2018 period. Major Diagnostic Category (ICD - 10) was used to define two discharge groups: 1. With normal term birth or 2. With HOR pathology. 1,658 and 1,669 egress from group 1 and 2 were compared respectively. The outcome variables are EM and AWDRG. Quantitative variables were compared with student t and Kruskal Wallis. Odds Ratio and respective confidence interval were used to evaluate association between variables and multivariate logistic regression to adjust association. RESULTS: Age, proportion of late pregnant women, MS and AWDRG was higher in HOR discharges (p <0.05). There is a strong association of prolonged MS (> 4 days) and elevated AWDRG (> 0.3109) with HOR discharges (ORa = 3.75; 95% CI 3.21-4.39 and ORa = 1.28; 95% CI 1.1-1.49 respectively). CONCLUSIONS: It is necessary to differentiate medical discharge of the maternity service because the HOR egress show greater complexity. The risk assessment using MS and AWDRG allows to analyze with specificity the maternal discharge for a better management of beds and human resources.

Background The Diagnosis Related Groups (DRG) constitute a method of classifying hospital discharges. Aim To report its development and implementation in a Chilean University Hospital and global results of 10 years Material and Methods We included 231,600 discharges from 2007 to 2016. In the development we considered the physical plant, clinical record flow, progressively incorporated human resources and computer equipment for coding and analysis to obtain results. The parameters used were: average stay, average DRG weight, mean of diagnosis and codified procedures, behavior of upper outliers, hospital mortality, distribution by severity and its relationship with other variables. Results The global complexity index was 0.9929. The average of diagnoses coded was 4.35 and of procedures was 7.21. The average stay was 4.56 days, with a downward trend. The top outliers corresponded to 2.25%, with stable hospital days and average DRG weight. The median of hospital mortality was 1.65% with a tendency to decrease and stable DRG mean weight. Seventy two percent had a grade 1 severity, with low median hospital stay. They occupied 40% of bed days. Nine percent had a grade 3 severity with high median hospital stay and accounting for 31.5% of bed days. Conclusions DRG methodology is a valuable information tool for decision making and result assessment in hospital management.

ABSTRACT

Objective: To describe the service use profile of Child and Adolescent Psychosocial Care Centers (Centro de Atenção Psicossocial Infanto-Juvenil [CAPSi]) in Brazil regarding diagnostic categories, sociodemographic aspects, and care modalities between 2008 and 2012. Methods: A descriptive, ecological study was performed using data from the Unified Health System regarding high-complexity procedure authorizations (Autorização de Procedimentos de Alta Complexidade [APAC]) for the period from 2008-2012. The variables sex, age, diagnosis (F00-F99 of ICD-10), and type of care provided were examined. The data were processed using TabWin and STATA version 12. Results: A total of 837,068 records were examined, each representing one visit to CAPSi. Most visits were by male users (68.8%). The most common diagnoses were hyperkinetic disorders (13%), pervasive developmental disorders (12.4%), and conduct disorders (8.4%). Conclusions: Behavioral and emotional disorders that usually appear during childhood or adolescence and psychological development disorders were frequent, with more than 50% of the latter comprising autism spectrum disorders. Regional differences were observed, with a higher presence of this diagnosis in the Southeast, while the North and Northeast had a high percentage of visits due to mental retardation.

Resumen Introducción Las enfermedades cerebrales vasculares son la primera causa específica de muerte y discapacidad en Chile. Objetivo Analizar características epidemiológicas de pacientes hospitalizados con diagnóstico de accidentes vasculares cerebrales en el Complejo Asistencial Víctor Ríos Ruiz. Metodología Estudio observacional, transversal. Se analizaron los egresos hospitalarios con diagnóstico de accidentes vasculares cerebrales entre 2014 y 2017. Se recolectaron datos sobre el diagnóstico de accidentes vasculares cerebrales mediante registro CIE 10, datos demográficos, tipo de accidentes vasculares cerebrales y datos respecto de la letalidad y estadía intrahospitalaria. Las variables cuantitativas fueron expresadas mediante promedios y desviación estándar (± DS) mientras que las variables categóricas fueron evaluadas mediante frecuencias absolutas y relativas. Las diferencias entre las variables fueron analizadas mediante T de Student y ANOVA de un factor. Se consideraron resultados significativos los valores p < 0,05. Resultados Se caracterizó un total de 1856 egresos, 58,6% fueron hombres, edad media de 66,9 (± 13,9) años y estadía media de 10,4 (± 16,7) días. En mujeres, la edad media fue 69,9 (± 16) años y estadía media 11,1 (± 16,5) días. El 55,5% correspondió a accidentes vasculares cerebrales isquémicos y 17,4% accidentes vasculares cerebrales hemorrágicos. Los principales factores de riesgo fueron hipertensión arterial (72%) y diabetes mellitus (33%). La letalidad intrahospitalaria global fue de 10,6%. Tanto la letalidad y estadía prolongada estuvieron asociadas a hemorragia subaracnoidea y accidentes vasculares cerebrales hemorrágicos (valor p < 0,05). Conclusiones La prevalencia de accidentes vasculares cerebrales es similar en hombres y mujeres. La hipertensión arterial fue el factor de riesgo más prevalente. A pesar de que el accidente vascular cerebral isquémico fue el más frecuente, la hemorragia subaracnoidea y el accidente cerebral vascular hemorrágico fueron relacionados con una mayor letalidad y estadía prolongada.


Abstract Introduction Stroke is the leading cause of death and disability in Chile. Objective To analyze the epidemiological characteristics of hospitalized patients with a diagnosis of stroke in our hospital unit (Complejo Asistencial Víctor Ríos Ruiz). Methods We performed an observational, cross-sectional study. We included patients who were discharged from our hospital with a diagnosis of stroke between 2014 and 2017. We extracted data on stroke-related ICD codes, demographic variables, types of stroke, case fatality rates, and hospital stay. Quantitative variables were expressed as averages with standard deviation (± SD), and categorical variables were expressed as absolute and relative frequencies. Differences were analyzed using Student t-distribution and ANOVA. We defined a p-value of < 0.05 as statistically significant. Results In total, 1856 patients were discharged of which 58.6% were male, with an average age of 66.9 (± 13.9) years, and an average stay of 10.4 (± 16.7) days. In the female population, the average age was 69.9 (± 16), and the average hospitalization was 11.1 (± 16.5) days. 55.5% of stroke cases was ischemic, and 17.4% was hemorrhagic. The main risk factors were hypertension (72%) and type 2 diabetes (33%). We found an overall in-hospital case fatality rate of 10.6%. Both the case fatality rate and prolonged in-hospital stay were associated with subarachnoid hemorrhage and hemorrhagic stroke (p < 0.05). Conclusions Prevalence of stroke is similar in both men and women. Hypertension was the leading risk factor associated with acute stroke. Although ischemic stroke was the most frequent diagnosis, both subarachnoid hemorrhage and hemorrhagic stroke were related to an increased case fatality rate and a more extended hospital stay.

Resumo OBJETIVO Avaliar as notificações e caracterizar os incidentes de quedas dos pacientes adultos internados em unidades clínicas e cirúrgicas de um hospital universitário na região sul do país, no período de 2011 a 2014. MÉTODO Estudo descritivo, transversal e retrospectivo, realizado no período de dezembro de 2016 a dezembro de 2017. A amostra foi de 1112 notificações, abrangendo todos os pacientes internados que foram notificados com ocorrência de quedas no período estudado. Os dados foram analisados por meio de estatística descritiva e analítica. RESULTADOS Foram predominantes na amostra os pacientes do sexo feminino e idosos, onde 69,4% dos incidentes não apresentaram dano. A ocorrência de quedas foi significativamente maior no período noturno. Limitação para deambular e estar desacompanhado foram os fatores mais prevalentes nas condições do paciente antes da queda. CONCLUSÃO Queda é um evento multifatorial que necessita avaliação periódica dos fatores de risco pela equipe para planejar sua prevenção.

MÉTODO Estudio descriptivo, transversal y retrospectivo, realizado en el período de diciembre de 2016 a diciembre de 2017. La muestra fue de 1112 notificaciones, abarcando a todos los pacientes internados que fueron notificados con ocurrencia de caídas en el periodo estudado. Los datos fueron analizados por medio de estadística descriptiva y analítica. RESULTADOS: Fueron predominantes en la muestra los pacientes del sexo femenino y ancianos, donde el 69,4% de los incidentes no presentaron daño. La ocurrencia de caídas fue significativamente mayor en el período nocturno. La limitación para deambular y estar desatendido fueron los factores más prevalentes en las condiciones del paciente antes de la caída. CONCLUSIÓN La caída es un evento multifactorial que necesita evaluación periódica de los factores de riesgo por el equipo para planificar su prevención.


Abstract OBJECTIVE To evaluate the occurrences and to characterize the falling incidents of adult patients hospitalized in clinical and surgical units of a university hospital in the southern region of the country, in the period from 2011 to 2014. METHOD Descriptive, cross-sectional and retrospective study, carried out from December 2016 to December 2017. The sample consisted of 1112 reports, covering all hospitalized patients who were notified with falls occurring in the studied period. Data were analyzed using descriptive and analytical statistics. RESULTS Female and elderly patients were predominant in the sample, in which 69.4% of the incidents did not present any damage. The occurrence of falls was significantly higher at night. Limitation to walking and being unaccompanied were the most prevalent factors in the patient's conditions before the fall. CONCLUSION The fall is a multifactorial event that requires periodic evaluation of the risk factors by the team to plan their prevention.

ABSTRACT

In the present study, the productions of antinociception induced by acute and chronic immobilization stress were compared in several animal pain models. In the acute immobilization stress model (up to 1 hr immobilization), the antinociception was produced in writhing, tail-flick, and formalin-induced pain models. In chronic immobilization stress experiment, the mouse was enforced into immobilization for 1 hr/day for 3, 7, or 14 days, then analgesic tests were performed. The antinociceptive effect was gradually reduced after 3, 7 and 14 days of immobilization stress. To delineate the molecular mechanism involved in the antinociceptive tolerance development in the chronic stress model, the expressions of some signal molecules in dorsal root ganglia (DRG), spinal cord, hippocampus, and the hypothalamus were observed in acute and chronic immobilization models. The COX-2 in DRG, p-JNK, p-AMPKα1, and p-mTOR in the spinal cord, p-P38 in the hippocampus, and p-AMPKα1 in the hypothalamus were elevated in acute immobilization stress, but were reduced gradually after 3, 7 and 14 days of immobilization stress. Our results suggest that the chronic immobilization stress causes development of tolerance to the antinociception induced by acute immobilization stress. In addition, the COX-2 in DRG, p-JNK, p-AMPKα1, and p-mTOR in the spinal cord, p-P38 in the hippocampus, and p-AMPKα1 in the hypothalamus may play important roles in the regulation of antinociception induced by acute immobilization stress and the tolerance development induced by chronic immobilization stress.

ABSTRACT

This study identified the explanatory power of the Korean rehabilitation patient group (KRPG) v1.1 for acquired brain injury (ABI) on medical expenses in the rehabilitation hospitals and the correlation of functional outcomes with the expenses. Here, the design is a retrospective analysis from the claim data of the designated rehabilitation hospitals. Data including KRPG information with functional status and medical expenses were collected from 1 January and 31 August 2018. Reduction of variance (R2) was statistically analyzed for the explanation power of the KRPG. Association between functional status and the medical expenses was carried out using the Spearman's rank order correlation (rho). From the claim data of 365 patients with ABI, the KRPG v1.1 explained 8.6% of variance for the total medical expenses and also explained 9.8% of variance for the rehabilitation therapy costs. Cognitive function and spasticity showed very weak correlation with the total medical expenses (rho = −0.17 and −0.14, respectively). Motor power and performance of activities of daily living were associated weakly (rho = −0.27 and −0.30, respectively). The KRPG and related functional status in ABI reflects the total medical expenses and rehabilitation therapy costs insufficiently in the designated rehabilitation hospitals. Thus, the current KRPG algorithm and variables for ABI may need to be ameliorated in the future.

PURPOSE: The aim of this study is to analyze inguinal hernia in Korea based on the nationwide data. METHODS: Retrospective analysis of nationwide data obtained from the Health Insurance Review & Assessment Service database for surgical repair of inguinal hernia from January, 2007 to December, 2015 was performed. RESULTS: A total of 314,238 cases of inguinal hernia surgery were performed during 9 years. The mean number of inguinal hernia surgery per year was 34,604 (range, 30,065–36,928). The proportion of male patient was 87%. Patients between the age of 1 to 5 years (17.3%) and of 60 to 69 years (17.4%) constituted the 2 dominant groups for inguinal hernia repair. A slight increase in repair for recurrent inguinal hernia, 1.9% to 3.2%, was observed from 2007 to 2015. The rate of concurrent bowel resection decreased from 1.9% to 0.6%. The total cost of inguinal hernia surgery was increased up to 1.68 times from 2007 to 2015 ($856 to $1,439), although the number of hospital days was decreased from 4.7 to 3.7 days. The rate of laparoscopic surgery was increased to 29.5% in 2015. CONCLUSION: A mean number of 34,604 cases of inguinal hernia surgery have been carried out annually in Korea. The age distribution for inguinal hernia repair demonstrates bimodal peak at early childhood and old age. The proportion of laparoscopic surgery of inguinal hernia has been increasing for the past 9 years. The length of hospital stay and the rate of combined bowel resection have decreased.

Resumen Introducción. La política farmacéutica de Colombia establece la necesidad de intensificar la investigación en farmacoepidemiología a nivel nacional, especialmente en el caso de los antibióticos. Objetivo. Aportar información farmacoepidemiológica en cuanto a la efectividad, las condiciones de uso y la seguridad de la cefepima y el meropenem genéricos en un hospital de alta complejidad en Bogotá. Materiales y métodos. Se hizo un estudio descriptivo, longitudinal y retrospectivo sobre la utilización de estos medicamentos. Los datos se recolectaron de todas las historias clínicas en las cuales se registraba el uso de cefepima y meropenem. Resultados. Se incluyeron 82 pacientes tratados con cefepima y 91 con meropenem. La mayoría de ellos había estado internada en servicios diferentes a la unidad de cuidados intensivos (59,8 % con cefepima y 52,7 % con meropenem). El 21,9 % de los tratados con cefepima y el 49 % de los tratados con meropenem, tuvieron consulta con un infectólogo, en tanto que en 47 % de los primeros y en 78 % de los segundos, se hizo cultivo o antibiograma. Las condiciones más frecuentemente tratadas con cefepima fueron las infecciones de vías respiratorias (32,5 %) y, con meropenem, las infecciones genitourinarias (34,8 %). Las tasas de éxito terapéutico fueron de 61,7 % para la cefepima y de 63,0 % para el meropenem. Conclusiones. Este estudio aporta información sobre el desempeño terapéutico de dos antibióticos genéricos de uso hospitalario. No hubo reportes de falla terapéutica durante el periodo de estudio. En los casos en que no hubo respuesta al tratamiento, las causas frecuentes fueron las alteraciones farmacocinéticas, las condiciones clínicas desfavorables y la elección inadecuada del tratamiento antimicrobiano.


Abstract Introduction: The Colombian national pharmaceutical policy establishes as a strategy the generation of greater pharmaco-epidemiological research at the national level, especially in the case of antibiotic drugs. Objective: To provide local pharmaco-epidemiological evidence regarding the effectiveness, conditions of use and safety of generic meropenem and cefepime in a tertiary hospital in Bogotá. Materials and methods: We conducted a descriptive, longitudinal and retrospective drug utilization study. The data were collected from the medical histories of all the patients who had cefepime or meropenem prescribed. Results: We included 82 patients treated with cefepime and 91 treated with meropenem in the study. Most of the patients were in services different from the intensive care unit (taking cefepime: 59.8%, and meropenem: 52.7%). Only 21.9% of the patients treated with cefepime and 49% of those treated with meropenem were seen by an infectious disease specialist. The antibiogram was performed for 47% and 60% of the patients treated with cefepime and meropenem, respectively. The most frequent indication for cefepime were respiratory infections and for meropenem, genitourinary ones. Therapeutic success rates were 61.7% for cefepime and 63.0% for meropenem. Conclusions: This study contributes evidence regarding the therapeutic performance of two generic antibiotics used in tertiary hospitals. There were no reports of therapeutic failure during the study period. In the cases of non-response, pharmacokinetic alterations, unfavorable clinical conditions, and inappropriate choice of antimicrobial treatment were identified as frequent factors.

Resumen Introducción: En 2009 la Influenza A H1N1pdm09 provocó en Chile 12.258 casos y 155 muertes. Objetivo: Analizar en adultos egresados de Clínica Dávila con influenza, en 2009, 2010, 2012 y 2014, soporte ventilatorio, costo de hospitalización, Grupos Relacionados por el Diagnóstico (GRD) y letalidad. Material y Método: Estudio descriptivo retrospectivo usando la ficha médica electrónica. Resultados: Egresaron 115.673 adultos, 338 (0,29%) con diagnóstico de Influenza, edad 56,5 ± 22 años, 59% mujeres, letalidad 4%. Hubo 3 grupos, Grupo 1: sin ningún soporte ventilatorio, 295 pacientes, edad 63 ± 20, estadía 6,6 ± 6,9 días, costo promedio de hospitalización $2.885.261, mediana peso GRD 0,41 (p25 = 0,38 y p75 = 0,62), letalidad 1,01% (3 pacientes). Grupo 2: Ventilación mecánica no invasiva (VMNI), 23 casos, edad 77,1 ± 13, letalidad 22% (5 casos), estadía 16,8 ± 12,4, costo $9.245.242, GRD 0,79 (p25 = 0,62 y p75=1,03). Grupo 3: Intubación y ventilación mecánica invasiva (VMI), 20 pacientes, edad 56,4 ± 15, estadía 36,9 ± 41,4, costo $38.681.099, GRD 5,86 (p25 = 5,82 y p75 = 5,86) y letalidad 30% (6 pacientes). Los GRD grupo VMI versus grupo VMNI y ningún soporte fueron diferentes (p < 0,0001 y p < 0,0001 respectivamente). La letalidad por influenza el 2014 fue de 8,5%, mientras que en los años 2012, 2010 y 2009 fue 1,5%, 3% y 2,5% respectivamente. La mediana de edad el año 2009 fue 37,5 años, menor que la de los otros años (p < 0,0001). Conclusiones: En 2009 los pacientes fueron más jóvenes, la necesidad de soporte ventilatorio provocó un peso GRD, estadía, costo y letalidad mayores que aquellos que no lo requirieron.


Introduction: In 2009 Influenza A H1N1pdm09 caused in Chile 12,258 cases and 155 deaths. Group 2: Non-invasive mechanical ventilation (NIMV), 23 cases, age 77.1 ± 13, lethality 22% (5 cases), stay 16.8 ± 12.4, cost 9,245,242 clp, DRG 0.79 (p25 = 0.62 and p75 = 1.03). Group 3: Intubation and invasive mechanical ventilation (IMV), 20 patients, age 56.4 ± 15, stay 36.9 ± 41.4, cost 38.681.099 clp, DRG 5.86 (p25 = 5.82 and p75 = 5,86) and lethality 30% (6 patients). The DRG group VMI versus group VMNI and no support were different (p < 0.0001 and p < 0.0001 respectively). The lethality for influenza in 2014 was 8.5%, while in 2012, 2010 and 2009 it was 1.5%, 3% and 2.5% respectively. The median age in 2009 was 37.5 yr.o significantly minor, than the other years (p < 0.0001). Conclusions: In 2009 the patients were younger, the need for ventilatory support led to a higher DRG weight, stay, cost and lethality than those who did not require it.

ABSTRACT

RESUMEN Objetivo Definir un modelo competencias profesionales para el desarrollo de un sistema de información de apoyo a la Gestión Clínica basado en Grupos Relacionados de Diagnósticos-GRD en hospitales públicos chilenos. Método Mixta. Investigación cualitativa, descriptiva, basada en entrevistas focalizadas con un muestreo teórico o intencionado a cuatro líderes expertos en GRD en Chile, con análisis de contenido; Investigación cuantitativa, con uso de Método Delphi a 18 gestores encargados de la implementación de las unidades de GRD en Chile, con 3 rondas. El análisis de los datos cuantitativos se realizó por conglomerados. Resultados Luego de cinco iteraciones, se evaluaron 78 competencias de un total de 179 en nivel "alto", del tipo: Conocimientos del líder de los equipos, formación profesional preferentemente enfermeras, 15 actitudes y valores, 17 habilidades o destrezas y 12 competencias específicas relacionadas al sistema de codificación. Conclusión Existe tendencia en los profesionales, a requerir el máximo de competencias, se observó una conducta masificadora, con baja discriminación y priorización. Se propone analizar las causas que dificultan la toma de decisiones y priorizar las competencias requeridas; Determinar para cada competencia el nivel requerido, las brechas entre la oferta de competencias y su demanda, y finalmente diseñar un sistema de evaluación del impacto del modelo en el desarrollo de competencias de los equipos.(AU)


ABSTRACT Objective To define a professional skills model for the creation of an information system to support clinical management based on diagnosis related groups (DRG) in Chilean public hospitals. Methods Mixed methodology. Qualitative, descriptive research based on focused interviews, with a theoretical or intentional sample of four leading DRG experts from Chile, with content analysis. Quantitative research using the Delphi method on 18 managers in charge of the implementation of DRG units in Chile, with three rounds. The analysis of quantitative data was carried out by clusters. Results After five iterations, 78 skills were evaluated out of a total of 179 as "high", including knowledge of the team leader, professional training (preferably nurses), 15 attitudes and values, 17 skills and 12 specific skills related to the coding system. Conclusion There is a tendency among professionals to require the maximum skills; a massive behavior was observed, with low discrimination and prioritization. To analyze the causes that make decision-making difficult and to prioritize the required skills is proposed to determine the necessary level for each skill, the gaps between skill offer and demand, and to design a system for evaluating the impact of the model on the development of the skills among the teams.(AU)

ABSTRACT

OBJECTIVE: This study examined the predictive factors for prolonged length of stays of adult patients with acute appendicitis (AA) in an emergency department (ED). METHODS: This was a retrospectively clinical study including patients in an ED. All patients were diagnosed from the clinical symptoms and a typical physical examination, and had undergone a computed tomography (CT) evaluation on the ED visiting date. All data were collected from the electrical medical records. The clinical parameters analyzed were the laboratory data, including the white blood cell count with differential values, C-reactive protein (CRP) level, initial vital signs, duration of admission, coexisting perforation of the appendix in the CT findings. The relationship between the clinical parameters and length of stay was assessed. RESULTS: A total of 547 patients with AA were enrolled in this study. Among them, there were 270 male patients with a mean age of 40.7±15.8 years. The baseline characteristics, initial clinical features, laboratory, and imaging studies results of 129 patients in the prolonged length of stay (pLOS) group, and 418 patients of the non-pLOS group in AA were compared. Multivariable logistic regression analysis revealed the predictive factors related to pLOS in AA to be as follows: age 40 years or older, body temperature over 37.3℃, CRP level greater than 5.0 mg/dL, and evidence of perforation in CT findings (P < 0.001). CONCLUSION: If we check age, fever, CRP level and find evidence of perforation, it might be helpful for predicting the increasing period of length of hospital stay for patients with AA in ED.
